Sapphire FUE Hair Transplant

Sapphire FUE is an advanced version of the standard FUE technique and represents one of the most important technological breakthroughs of recent years in the field of hair transplant. While these three stages are conducted independently of one another in standard FUE and Sapphire FUE, in DHI the follicle is placed directly into the recipient area with the implanter device immediately after being extracted from the body. This simultaneous application minimizes the time the follicles spend outside the body, increasing the graft survival rate. Unshaved Hair Transplant

Unshaved hair transplant is, as its name suggests, a procedure performed without the need to shave the donor area. While the donor area must be completely or largely shaved in standard FUE and Sapphire FUE applications, in the unshaved technique each follicle is extracted individually from between the long hairs. This method offers an extremely attractive option for individuals who do not wish to experience a noticeable change in appearance after surgery, who want to keep their hair transplant discreet, and who need to return to their social life immediately after the procedure. However, the unshaved technique requires far more attention, experience, and time on the part of the surgical team; for this reason, the procedure duration is longer compared to standard techniques and graft count limitations may generally apply.

Pre-Operative Preparation for Hair Transplant

The preparation process before hair transplant is a critical stage that directly affects the success of the procedure and the quality of healing. The care shown during this process both increases surgical safety and enhances the quality of the results.

At least 2 weeks before the procedure, the use of blood-thinning medications, aspirin, omega-3 supplements, and some herbal products should be evaluated with the physician; if necessary, their use should be discontinued. Alcohol consumption should be stopped at least 1 week before the procedure; alcohol both has a blood-thinning effect and negatively affects the healing process. Since smoking is known to impair tissue oxygenation and negatively affect the graft adherence rate, stopping or significantly reducing smoking at least 1 week before the procedure is recommended. Hair dye, keratin, and chemical treatments should not be applied at least 2 weeks before the procedure. On the day of the procedure, hair should be washed and arrived clean without any products. Comfortable clothing that buttons or zips at the front should be preferred; because clothing that passes over the head after the procedure can damage the grafts.

How Is Hair Transplant Performed?

The hair transplant procedure consists of four fundamental consecutive stages. Depending on the technique selected, the order and details of these stages may vary.

In the consultation and planning stage, the physician determines the hairline, the donor area, and the number of grafts to be transplanted; an individual operation plan is created taking into account the person’s facial features and aesthetic expectations. In the anesthesia stage, the donor and recipient areas are numbed with local anesthesia; the patient is completely awake throughout the procedure and feels no pain. In the graft extraction stage, follicles are extracted from the donor area one by one or in strip form according to the selected technique and kept viable in special preservation solutions. In the channel opening and placement stage, channels are opened in the recipient area in accordance with the natural growth angle and direction of the hair; the follicles are placed one by one into these channels and the operation is completed. The duration of hair transplant varies between 6 and 10 hours depending on the number of grafts to be transplanted.

Post-Operative Care After Hair Transplant

The care protocol applied after hair transplant is the most critical process that directly determines the graft adherence rate and the quality of the final result. Meticulously adhering to the rules during this period is as important as the surgery itself for the success of the hair transplant.

Keeping the head upright on the first night and using a special neck pillow are of great importance for minimizing swelling. For the first 3 days, the transplanted area must absolutely not be touched, contacted, or subjected to pressure. For the first 10 to 15 days, the transplant area should be protected from rain, sunlight, and dust. The initial washing protocol determined by the clinic must be meticulously followed; the first wash is generally performed by the clinic 2 to 3 days after the procedure. Smoking and alcohol use must be completely stopped for the first month. Swimming pools, the sea, saunas, and Turkish baths are strictly prohibited for the first 1 month. Intense exercise and heavy physical activity should be postponed for the first 2 weeks. The use of hats and beanies should be restricted for the first 2 weeks; when necessary, they should be worn in a manner that does not come into contact with the transplanted area. The use of sunscreen is critically important for the transplant area for the first 3 months.

Recovery Process After Hair Transplant

The recovery process after hair transplant is a gradual process that requires patience. Knowing the changes that occur at each stage in advance both eliminates unnecessary anxiety and allows for approaching the process with accurate expectations.

In the first few days, redness, swelling, and crust formation are observed in the transplanted area; these symptoms are completely normal and part of the expected healing response. The swelling in the forehead area reaches its most pronounced state on days 3 to 5 and then begins to gradually subside. Within the first 10 to 14 days, the crusts fall off and the healing process is visually largely complete. The redness in the donor area disappears within 1 to 2 weeks.

In the first 2 to 3 weeks, the transplanted hairs begin to adhere, though no visually noticeable change is yet observed. Between weeks 3 and 4, a process called shock shedding begins, which concerns many patients; the majority of the transplanted hairs shed, leaving only the follicles in place. Shock shedding is a completely normal and expected process; the follicles are healthily positioned and are preparing to regrow according to their own growth cycles.

Hair Transplant Results: Detailed Month-by-Month Progress

Months 3 to 6

The 3rd to 6th months after hair transplant are the period during which the first sparks of new hair growth begin to be seen and exciting developments take place. After the completion of shock shedding, from the 3rd month onward the follicles begin to enter the active growth phase and new, fine, downy hair strands begin to emerge to the surface. During this period, the hairs are still very fine and short in structure; this downy appearance, far from full density and thickness, does not reflect the final result. By the end of the 6th month, between forty and sixty percent of hair density has become evident and the first genuine evaluation can be made during this period. At this stage, the appearance obtained does not yet reflect the final result; however, it becomes possible to see the general profile of the outcome to be achieved.

Months 6 to 9

The 6th to 9th months are the most exciting stage of the hair transplant process, during which the most prominent transformations take place. During this period, hair strands continue to thicken and lengthen rapidly; hair density and overall appearance improve noticeably with each passing month. In the 7th month, the hairs have now shed their downy appearance and are approaching real hair texture; density has increased noticeably and the hairline has begun to sharpen. In the 8th month, the vast majority of hairs are in the active growth phase and approximately seventy percent density has been achieved; during this period, many patients begin to receive positive comments from their social circle and loved ones for the first time. By the end of the 9th month, in the vast majority of cases between eighty and ninety percent of hair density has taken its place. During this period, the hair can be styled, adapted to desired looks, and begins to present an aesthetically complete appearance. The donor area has fully healed and all marks have become largely invisible.

Months 9 to 12 and Beyond

The 9th to 12th months represent the maturation period of hair transplant, during which the final steps toward achieving the ultimate result are taken. Hair strands reach their thickest, strongest, and healthiest state during this period; full density and full naturalness are achieved. In the 10th month, the transplanted hairs have become visually indistinguishable from the other hairs, and the hairline, forehead area, and crown area have attained the desired aesthetic appearance. In the 11th month, the hairs have entered long-term growth cycles; they behave, are cut, and can be styled exactly like completely normal hair. In the 12th month and beyond, the final result has taken its place and photographic evaluations made during this period clearly demonstrate the permanent effect of the hair transplant. In some individuals, full maturation may take between 12 and 18 months; this depends on the individual’s growth rate and follicle structure. From this period onward, the transplanted hairs will continue to grow for a lifetime.

A transplant performed while active shedding continues may bring with it the risk of the existing hairs shedding in addition to the transplanted ones. The quality and capacity of the donor area is also a critical factor; in transplants performed with an insufficient donor reserve, the desired density may not be achieved.
